Libido – or sexual desire – is a fundamental part of health and well-being, influenced by various physical and emotional factors. As the years go by, both men and women can experience a decrease in libido, often caused by natural hormonal imbalances that accompany aging, such as menopause and andropause. The good news is that hormone replacement therapy can help restore sexual desire and pleasure, improving quality of life. In this article, we will explore how hormone replacement therapy impacts libido and in what situations it may be beneficial.

1. How Hormones Affect Libido

The relationship between hormones and sexual desire:
Sex hormones, such as testosterone, estrogen, and progesterone, play a vital role in regulating sexual desire and pleasure. In both men and women, levels of these hormones naturally begin to decline from around age 30, but it is the onset of menopause or andropause that causes a more pronounced drop, directly affecting sexual desire and response to pleasure.

Impact on women and men:
In women, estrogen and progesterone play an important role in vaginal lubrication and sexual sensitivity. In men, testosterone is the main hormone associated with sexual desire and physical vitality. A lack of these hormones can lead to symptoms such as vaginal dryness, loss of sexual interest, and difficulty maintaining arousal, in both women and men.

2. Hormone Replacement Therapy and Libido: How Does it Work?

The role of hormone replacement therapy (HRT and TRT):
Hormone replacement therapy is a way to restore hormonal levels to their natural balance, helping to alleviate symptoms associated with a lack of sex hormones. In hormone replacement therapy for women, estrogen and progesterone are used to reduce menopausal symptoms like vaginal dryness, hot flashes, and loss of libido. For men, testosterone replacement therapy (TRT) is indicated to treat low testosterone and associated symptoms, such as fatigue, loss of muscle mass, and decreased sexual desire.

Direct effects on libido:
Studies show that hormone replacement therapy has a positive impact on libido and sexual satisfaction. In women, HRT can improve vaginal lubrication and sensitivity, facilitating arousal. In men, TRT helps increase sexual desire and performance. Additionally, hormone replacement therapy can also improve mood and reduce stress, factors that directly influence libido and the quality of relationships.

3. Emotional and Psychological Benefits of Hormone Replacement Therapy

How emotional health affects libido:
Libido is strongly linked to mental and emotional health. Factors such as stress, anxiety, and depression can affect the quality of intimate relationships and reduce sexual interest. Hormonal imbalances, in addition to impacting the body, can influence mental well-being, causing irritability, mood swings, and discouragement.

Hormone replacement therapy helps promote well-being:
By restoring hormonal levels, hormone replacement therapy contributes not only to improving sexual desire but also to promoting greater emotional balance. Studies show that individuals undergoing hormone replacement therapy experience significant improvements in self-esteem, mood, and quality of life, creating a more conducive environment for pleasure and sexual satisfaction.

4. When is Hormone Replacement Therapy Indicated?

Medical evaluation and eligibility criteria:
Not everyone needs hormone replacement therapy. To determine the need and safety of the treatment, a thorough medical evaluation is essential. Hormone replacement therapy is indicated for men and women presenting symptoms of hormonal deficiency who would benefit from treatment. In women, it is more common after menopause. For men, testosterone levels must be below normal for TRT to be indicated.

Continuous monitoring:
Hormone replacement therapy is an effective and safe treatment when monitored appropriately. The correct dosage is adjusted as needed, and medical follow-up is essential to ensure the best outcomes and minimize potential risks.

Hormone replacement therapy is a valuable alternative for those struggling with decreased libido and other symptoms of hormonal imbalance. By restoring hormonal levels, it offers physical and emotional benefits that improve quality of life and bring back pleasure and intimacy in relationships. If you notice changes in sexual desire and believe they may be related to hormonal levels, consult a specialist to evaluate your options and discover if hormone replacement therapy is right for you.

Hormone therapy is a versatile and powerful treatment for various health conditions. It involves the controlled use of hormones to rebalance the body and relieve symptoms associated with hormonal disorders. Besides being widely known for treating menopause symptoms, hormone therapy is effective for a variety of other diseases. In this article, we will explore four conditions that can be treated with hormone therapy, helping to clarify how this approach can improve the health and quality of life of patients.

Besides being widely known for treating menopause symptoms, hormone therapy is effective for a variety of other diseases. In this article, we will explore four conditions that can be treated with hormone therapy, helping to clarify how this approach can improve the health and quality of life of patients.

1. Hypothyroidism

What it is:
Hypothyroidism occurs when the thyroid gland does not produce enough hormones to meet the body’s needs. This condition affects metabolism, leading to symptoms such as fatigue, weight gain, dry skin, and cold intolerance.

How hormone therapy helps:
Hormone replacement with levothyroxine, a synthetic form of the T4 hormone, helps restore hormonal balance by replacing the hormone that the thyroid cannot produce. With the correct dosage, hormone therapy for hypothyroidism is safe and effective, improving symptoms and helping the patient maintain a healthy metabolism.

2. Menopause and Its Symptoms

What it is:
Menopause is a natural phase of life, marked by a decline in female hormones, primarily estrogen and progesterone. This change brings symptoms like hot flashes, night sweats, mood swings, insomnia, and vaginal dryness.

How hormone therapy helps:
Hormone replacement therapy (HRT) can alleviate these symptoms by restoring levels of estrogen and progesterone in the body. HRT is adjusted according to each patient’s hormonal profile and, when well-monitored, reduces symptoms, improves bone health, and decreases the risk of certain cardiovascular diseases.

3. Male Hypogonadism

What it is:
Male hypogonadism is a condition in which the body does not produce enough testosterone, which can impact sexual health, muscle mass, bone density, and overall well-being. This condition may be caused by genetic issues, injuries, or diseases affecting the testes or hypothalamus.

How hormone therapy helps:
Testosterone replacement therapy (TRT) is indicated for men with diagnosed hypogonadism and works by raising testosterone levels to a normal and healthy range. TRT can enhance energy, boost libido, strengthen bones and muscle, and provide benefits for mood and quality of life.

4. Osteoporosis

What it is:
Osteoporosis is a disease that weakens bones, making them more fragile and susceptible to fractures. This condition is more common in women after menopause, due to the decline in estrogen levels, which plays a crucial role in bone health.

How hormone therapy helps:
Estrogen replacement can help strengthen bones and reduce the risk of fractures, especially in postmenopausal women. HRT is considered one of the options to prevent bone loss, although it should only be indicated after careful analysis of risks and benefits. For some patients, especially those who cannot take other types of medication for bones, HRT can be an excellent option.

Hormone replacement therapy
is a powerful ally for the treatment of various health conditions, providing balance and relief from symptoms for many people. Each treatment should be personalized and closely monitored by a healthcare professional to ensure its effectiveness and safety. If you suffer from any of these conditions and believe that hormone therapy could be helpful, talk to your doctor to explore the available options and discover how this treatment can improve your health and quality of life.

Blue November is a global awareness campaign focused on the importance of early diagnosis of prostate cancer. Throughout the month of November, the campaign incentivizes men to take care of their health and undergo essential preventive examinations.

Prostate cancer is the second most common type of cancer among men, only behind skin cancer. In Brazil, a man dies from this disease every 38 minutes. Despite this, there is still significant controversy over whether Testosterone Replacement Therapy (TRT) can increase the risk of prostate cancer. However, several studies already suggest that this relationship is unlikely.

Recently, research published in the prestigious JAMA thoroughly examined the effects of TRT concerning prostate cancer and other prostatic events. The study, titled “Prostatic Safety Events During Testosterone Replacement Therapy in Men with Hypogonadism”, was a randomized, double-blind clinical trial with 5,246 participants. It compared the effects of TRT with a placebo in men aged between 45 and 80, all with low testosterone levels (below 300 ng/dL). Men with elevated PSA levels (>3.0 ng/mL) or severe urinary symptoms were excluded.

Main Conclusions from the Testosterone and Prostate Health Study

  1. Prostate Cancer Risk: There were no significant differences in the incidence of prostate cancer between the TRT and placebo groups, indicating that testosterone hormone therapy does not increase the risk of high-grade cancer.
  2. Secondary Events and Urinary Symptoms: The incidence of acute urinary retention, procedures for benign prostatic hyperplasia (BPH), and treatments for urinary symptoms also did not differ significantly between the groups. PSA levels increased slightly in the TRT group but stabilized after 12 months, with no significant progression of urinary symptoms.
  3. Monitoring and Safety: This is one of the largest randomized studies on TRT and prostate cancer to date. The evidence is robust and supports that hormone therapy does not increase the risk of serious prostate-related events in men carefully selected for treatment.

Despite the positive evidence, the study has limitations. Individuals with a history of prostate cancer and elevated PSA levels were excluded, and the 33-month follow-up, while extensive, may not be sufficient to fully assess the long-term effects of TRT. Another factor was the high discontinuation rate in both the TRT and placebo groups, primarily due to the COVID-19 pandemic, which affected participant follow-up.

Final Considerations on Hormonal Therapy and Blue November

This study reinforces the safety of Testosterone Replacement Therapy in men with hypogonadism, providing additional support for physicians and patients to make more informed treatment decisions. Although TRT does not seem to increase the risk of prostate cancer, careful patient selection and continuous monitoring are essential to ensure the safety and effectiveness of testosterone hormone treatment.

During Blue November, in addition to focusing on early diagnosis, raising awareness about treatments such as TRT and their impact on men’s health is essential to promote a more comprehensive and safe approach to men’s health.
